Hormone Replacement Therapy



As we age, decreasing levels of sex hormones can cause health problems for both men and women. Declining levels of testosterone in older men can cause symptoms such as fatigue, loss of muscle strength and mass, and decreased libido. Testosterone replacement therapy has become increasingly popular in recent years due to its promising results. Testosterone replacement can be accomplished in several ways, including transdermal creams or gels, troches or injections. All of these dosage forms release testosterone into the bloodstream, which protects it from degradation in the digestive tract. The creams, gels and troches made at Talon Compounding Pharmacy are formulated with bioidentical testosterone, which is the same in structure as that produced by the testes. Injectable testosterone is a commonly-prescribed dosage form, since, unlike other dosage forms, it has the advantage of once-weekly administration. This is possible because the testosterone salt forms used to make the injections (usually cypionate, enanthate or propionate) allow the testosterone to be released over a longer period of time. The testosterone injections made in TCP's sterile facility are formulated in a unique thin oil, which eliminates the need for needle switching and makes injections more comfortable.

Traditionally, perimenopausal symptoms in women have been treated mainly with gels or tablets containing hormones, some synthetic, in set dosage amounts. Although often effective at relieving the symptoms of perimenopause, these medications are associated with a number of side effects, some of them serious. Bioidentical hormone replacement therapy (BHRT) arose as an alternative to synthetic hormone replacement. The hormones used in BHRT are yam-derived, and are structurally identical to the hormones produced by the ovaries. Although there are no head-to-head studies comparing bioidentical hormones to synthetic hormones (due to a lack of funding to complete such a study), the thinking among many practitioners who specialize in hormone replacement is that using bioidentical hormones may be less disruptive to the body, which may avoid many of the side effects seen with traditional hormone replacement. An additional advantage of compounded BHRT is that they are highly customizable. They can be formulated into creams, gels, troches, capsules or suppositories depending on patient and practitioner preference. Different hormones can be combined into one preparation, and an almost limitless range of strengths allows us to tailor therapy to every patient’s individual needs.
